Information that you provide during treatment is confidential and will not be released to persons outside the HHS or the UCC unless you sign a statement authorizing such a release. Separate files will be maintained for each partner participating in couples treatment. The UCC includes the Learning Assistance Program (LAP), Outreach and Prevention Program (OPP), Stress Management Program (SMP), and Drugs, Alcohol, and You Programs (DAY). Exceptions to confidentiality as mandated by law are as follows: imminent danger to yourself or others, child/elder abuse, grave disability, subpoenaed records, and threat to national security. In addition, information that you provide to the HHS and the UCC will be stored in a secured electronic format. Any digital recordings of treatment are not considered to be a part of your clinical record. The digital records are used exclusively for supervision purposes and will be erased after use. This information is utilized for administrative purposes and to aid staff in delivering the highest quality health services to our clients. Information you choose to provide via e-mail or the internet may not be accessed and read immediately and cannot be guaranteed confidential.

If you are interested in visiting the UCC for personal concerns, please come in between the hours of 10am and 3pm. You will be asked to complete paperwork and then you will have a brief, confidential meeting with the triage counselor who will help you determine the most appropriate and services or resources for your particular concerns. These services and resources may be best provided at the UCC or in the campus/Fort Collins community.
